Quarterly is enough. Keep the survey short so people will actually complete it, then compare results over time. Use company Google sign-in to limit responses to one per person, while keeping names and emails out of the reported results.

Open text response. Do not include names, client-sensitive details, confidential information, or anything that could identify another person.

The Cloudflare page shows aggregated quarterly results from submitted responses. Names and emails are not displayed in the dashboard.

    Privacy stance

    Anonymous means anonymous.

    Require company Google sign-in only to prevent duplicate submissions. Do not show names or emails in the dashboard, and do not report individual responses. If the team wants follow-up stories later, ask for volunteers separately.

    Principle

    One response per person, anonymous reporting.

    Use Google sign-in as a gate against duplicate responses, then aggregate the results before sharing. The results should improve guidance, training, and tool access — not evaluate individual performance.
